APPLICANT'S STATEMENT OF AUTHORIZATION
I hereby affirm that the information provided on this application (and accompanying resume, if any) is true and complete to the best of my knowledge. I also agree that any falsified information or significant omission(s) may disqualify me from further consideration for employment and may be considered justification for dismissal at a later date if I am employed by the Company.
I authorize a thorough investigation of my past employment and activities, and I agree to cooperate in such investigations. I authorize the Jewish Association on Aging to contact my prior employers and all others (with the exception of my current employer if I have marked "No" in response to the question ''May we contact your present employer?" on page one (1) of this application) for the purpose of verification of the information I have supplied.
To the fullest extent permitted by law, and after receiving a conditional offer of employment, I further authorize any physician, hospital or health care provider to release any information which may be necessary to determine my ability to perform the essential functions of the job for which I may be considered or any future job in the event that I am hired. I understand that I must meet the qualifications for the position offered, with or without reasonable accommodation, before being permitted to commence work with the Jewish Association on Aging.
I further agree to inquiries into my character that relate to the job for which I am applying, including, but not limited to credit and criminal history, as well its diplomas, degrees, licenses, transcripts, etc. which are relevant to an evaluation of my qualifications for employment. I understand that a separate disclosure and consent form will be provided to me prior to any bacground check.
I hereby agree that after I receive a conditional offer of employment I will submit to any lawful drug, alcohol or psychological/integrity testing that may be required as a condition of employment, or continued employment, and understand that refusal to submit to such testing may result in disciplinary action if I am employed by the Company, up to and including discharge, or withdrawal of a conditional offer.
I understand that I must receive a negative test result for illegal drug use before being permitted to commence work with the Company.
I understand that any offer of employment is contingent upon satisfactorily passing the Employer's physical examination (including drug testing). I also understand that as a condition of continued employment, I agree to participate in any required annual physical examinations as well as all required in-service training sessions.
I understand that, if I am hired, my employment is terminable-at-will, that I am not being employed for any specified period of time, and that this application is not intended to be a contract for continued employment.
I understand that according to federal law, all individuals who are hired must, as a condition of employment, produce certain documents to verify their identity and U.S. citizen status or, if aliens, their legal authorization to work in the U.S.
As a consequence, I understand that any offer of employment would be contingent upon my ability to produce the required documentation within the time period required by law.
I hereby release the Jewish Association on Aging, its affiliated agencies, any of my references and my former employers from any liability for any damage due to releasing information regarding me.
I understand that this application will expire in 90 days. After that date, unless otherwise notified, I understand that my status as an applicant will end. I may re-apply for employment by completing a new application.
